The homepage names White Label MD LLC and WLMD Network for clinical services and Manifest Pharmacy as the compounder. These disclosures are starting points, not confirmation of your assigned clinician or dispenser. The site identifies the compounded medicine as not FDA-approved. The telehealth consent allows care from a nurse practitioner or physician assistant. Remote care may also lead to an in-person referral or a decision against prescribing. If continuity with a particular clinician matters to you, establish that before enrolling.

Moving from another provider

A membership refund is not a medication refund

The terms promise a membership-fee refund if treatment is declined, and a prorated membership refund for the unused part of the current 28-day cycle. Shipped compounded medication is nonrefundable.

Review privacy before sending health information

The privacy policy assigns the professional entities their own privacy-notice responsibilities and says the platform is not a HIPAA covered entity. Its introduction uses the FMMeds trading name although the notice covers boostAGErx. Clarify which notice applies before sending health information; that inconsistency does not establish that the brands are the same business or that clinical care lacks HIPAA protections.
