Zepbound and CPAP: What the Trials Cannot Decide
Separate Zepbound’s OSA trial results from the unresolved decision about ending PAP, including temporary research withdrawal and residual disease.
On this page
Zepbound’s sleep-apnea evidence leaves a specific question open: when an established positive airway pressure (PAP) user can safely stop that treatment. The label explicitly says the trials did not evaluate the timing or appropriateness of PAP discontinuation in previously compliant users.1
Keep the approved population in view
FDA’s December 2024 approval covers moderate-to-severe obstructive sleep apnea in adults with obesity, alongside reduced-calorie eating and increased activity. Our Mounjaro–Zepbound comparison explains why shared tirzepatide does not erase product-specific approved-use limits.2
The two PAP groups were not randomized against each other
The protocol assigned people to a study according to existing PAP use. One group was unable or unwilling to use PAP; the other had used it for at least three months and planned to continue during the study. Randomization then compared tirzepatide with placebo within those populations. Comparing the two trial averages therefore cannot tell you whether medication should replace PAP for an individual.4
A measurement interruption was part of the research
The PAP trial used seven-day interruptions before assessments to limit PAP’s effect on the measurements. That scheduled research procedure did not establish permanent freedom from PAP or a schedule for readers.5
Participants with type 1 or type 2 diabetes, or central or mixed apnea, were excluded.3
Read the combined endpoint
At week 52, 42.2% and 50.2% of tirzepatide recipients in the non-PAP and PAP studies met a combined endpoint: AHI below five, or AHI five to fourteen with Epworth Sleepiness Scale at most ten. AHI counts breathing events per hour. Mild residual disease qualifies; this is not a PAP-free percentage.1
Our benefits overview places sleep-apnea findings in broader context.2
The 2024 FDA review preserved the uncertainty
FDA’s 2024 review said the trials did not establish whether previously compliant PAP users should discontinue treatment. It called for further evidence. Improved measurements do not determine an individual’s remaining disease, symptoms or future PAP requirement.5
CPAP is the fixed-pressure form of positive airway pressure (PAP) therapy.5
Fifty-two weeks was the treatment period
The protocol planned four weeks of screening, 52 weeks of treatment and four weeks of post-treatment follow-up. The short follow-up period should not be described as evidence of years of successful PAP discontinuation. It also does not turn the original allocation by PAP use into a randomized test of long-term withdrawal. The trial’s calendar helps define what was actually observed.4
Make reassessment a question for the sleep-treatment team
The 2021 AASM guidance supports considering follow-up sleep testing after clinically significant weight change, while discouraging routine retesting of asymptomatic PAP users. Recurrent or persistent symptoms despite good PAP adherence can also support reassessment. It leaves the specific judgment to the clinician. A useful reassessment discussion includes the original diagnostic study, current PAP data, symptoms and weight history, rather than a target weight alone. If care is split between services, our provider-switch guide helps organize record transfer. This article does not establish an individual testing schedule or PAP change.6
