Mounjaro vs. Zepbound: Uses, Ages and Dose Limits

Mounjaro and Zepbound contain tirzepatide and share a 15 mg weekly maximum for adults. That one matching number does not make their labels identical: pediatric Mounjaro has a lower maximum, and the brands have different approved uses.^{1, 2}

The practical starting point is the condition being treated, the patient's age and the exact prescription. Compare those before choosing a service or assuming that a familiar weight-loss percentage describes the expected result of every tirzepatide prescription.

Mounjaro includes pediatric diabetes and an adult cardiovascular indication

Mounjaro is approved with diet and exercise to improve glycemic control in adults and children aged 10 and older with type 2 diabetes. Its cardiovascular indication is separate: reducing major cardiovascular-event risk in adults with type 2 diabetes at high risk for those events.¹

The pediatric indication is not an approval for childhood weight management or pediatric cardiovascular risk reduction. Nor does a type 2 diabetes indication establish suitability for someone without that diagnosis. The treating practice should name the indication it is addressing.

Zepbound covers adult weight management and specific sleep apnea

Zepbound is approved with a reduced-calorie diet and increased physical activity for long-term weight management in eligible adults. Its separate sleep indication concerns moderate-to-severe obstructive sleep apnea (OSA) in adults with obesity. It is not a general approval for every form of sleep apnea or every person who snores.²

Zepbound's reviewed label does not establish pediatric safety and effectiveness. Its OSA indication also does not mean someone should stop positive airway pressure (PAP) treatment on their own. The sleep-treatment plan needs reassessment by the clinician managing it.

Adult maxima match, but maintenance instructions do not

Mounjaro's maximum is 15 mg once weekly in adults and 10 mg once weekly in pediatric patients. Its label describes individualized escalation when additional glycemic control is needed.¹

Zepbound specifies 5 mg, 10 mg or 15 mg weekly maintenance for weight management, and 10 mg or 15 mg for OSA. The 2.5 mg starting dose is not a maintenance dose.² Copying the weight-management maintenance list onto every Mounjaro patient or every OSA prescription would misstate those instructions.

These are distinctions in the labels, not a plan for choosing or changing a dose. The prescriber should explain the intended regimen, tolerability checks and follow-up. The weight-loss trial was not a universal prediction

SURMOUNT-1 studied adults with obesity, or overweight with a weight-related complication, excluding type 2 diabetes. At 72 weeks, mean weight changes with 5 mg, 10 mg and 15 mg tirzepatide were −15.0%, −19.5% and −20.9%, compared with −3.1% for placebo; treatment included lifestyle intervention.⁴

The sleep-apnea evidence comes from two different groups

SURMOUNT-OSA included two 52-week trials in adults with obesity and moderate-to-severe OSA, without type 2 diabetes. One enrolled people unable or unwilling to use PAP; the other enrolled PAP users. The placebo-adjusted reductions in apnea-hypopnea index were 20.0 and 23.8 events per hour, respectively.³

The first number is not a pooled result for every patient. Lower average event counts do not establish that every participant's OSA resolved or that PAP can be discontinued. This evidence supports a specific clinical discussion, rather than replacing the sleep clinician's assessment.

A shared ingredient does not settle preparation or precautions

Both products are approved tirzepatide injections. A separately compounded preparation is not either branded finished product; FDA does not approve compounded drugs or review them for safety, effectiveness and quality before marketing.⁵ See our compounded-versus-brand guide before treating a product name as an equivalence claim.

Tell the prescriber about other medicines and contraception. The tirzepatide labels advise oral hormonal contraceptive users to switch to a nonoral method or add a barrier method for four weeks after starting and after each dose escalation.^{1, 2} Our birth-control guide explains the product-specific instructions and evidence limits.

Compare access after the prescription is clear

Ask which product, indication, age-specific regimen and presentation are proposed. Then confirm the actual pharmacy quote, care fee and benefit requirements. Approved indications do not guarantee coverage under an individual insurance plan; our insurance guide covers the questions to ask.
